Job Description :
1. Assess, plan, organize and participate in rehabilitation programs that improve mobility, relieve pain, increase strength and decrease or prevent deformity or patients suffering from disease or injury.
2. Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
3. Plan, prepare and carry out individually designed programs of physical treatment to maintain, improve or restore physical functioning. Alleviate pain and prevent physical dysfunction in patients.
4. Record prognosis, treatment, response and progress in patient’s chart or enter information into computer. Our facility is outpatient orthopedic with specialization in pelvic floor therapy, vestibular rehab and lymphedema care. Experience in and/or certification in any of those specialized areas is preferred.
Required Skills : Master’s or Doctoral Degree in Physical Therapy; Indiana license to practice as a physical therapist with current certification if working as a Physical Therapist. Indian license to practice as a Physical Therapist Assistant with current certification if working as a PTA.
